In registering for Stress Down Day and submitting this form, I acknowledge that I understand the activities and risks involved in participating in Stress Down Day; and agree, inconsideration of permission to participate in the activities, to release and indemnify Lifeline, its officers, employees and volunteers and all sponsors (be they individual or organisations, singularly or collectively) from and against all liabilities, claims, damages, suits, expenses, causses of action, injuries, losses or inconvenience of any description whatsoever arising in any way from my participation (or the participation of the person names) in the activities. In submitting this form I confirm that I am over the age of eighteen (18) years old, or, as I am under the age of eighteen (18) years I have obtained permission to participate from my parent/guardian or school official. I also give permission for the free use by Lifeline of my name, image and voice in any broadcast, or another account in any media, of Lifeline.

Friday 23 July is Lifeline's Stress Down Day and if you visit your local library, you'll see staff in comfy casuals, PJs and slippers. Getting involved is as easy as wearing your slippers to work or school, dressing up or down on Stress Down Day. Every minute a new call is made to Lifeline's 24 hour 13 11 14 crisis support service. The money raised from donations will help pay for Lifeline's $1.4 million annual phone bill and train more telephone crisis support volunteers with the skills to answer calls from Australians in need of support. Lifeline's 13 11 14 crisis suport service has been answering calls from Australians in need of help for over 47 years, so now we are asking you to THROW US A LIFELINE and make a donation.
